This policy covers information collected by Pressure Medical Technology, LLC (PMT) through its websites, inquiry forms, and SMS programs.
1. Information We Collect
When you submit a form, we collect the information you provide, which may include your name, email address, phone number, job title, organization, facility type and location, chamber interests, purchase timeline, project or support details, referral contact information, and support attachments. SMS-related information includes your number, consent choices, messages, and opt-out requests. Website visits may generate technical information such as IP address, browser type, pages visited, and referring URL through cookies and similar technologies.
Our forms are intended for equipment and project inquiries. Do not submit protected health information or personal medical information. These forms are not designed as HIPAA-compliant channels for health data.
2. How We Use Information
We use information to respond to inquiries, coordinate consultations, provide requested project and service updates, manage referrals, maintain customer communications, improve our website, and comply with applicable law. We do not sell personal information. Submitting a form or providing a phone number does not by itself give permission for SMS.
3. SMS Consent and Your Choices
At our request-pricing form, the first optional, unchecked SMS checkbox authorizes customer-care messages about pricing or service inquiries, consultation scheduling, and requested project or service updates. A separate optional, unchecked checkbox authorizes marketing messages such as special offers, discounts, and product updates. Each consent applies only to its stated message category. Neither checkbox is required to purchase.
Message frequency varies. Message and data rates may apply. Reply STOP to unsubscribe or HELP for help. You may also contact [email protected] or (602) 688-9255. A single opt-out confirmation may follow a STOP request. Further texts require a new opt-in. See our SMS Terms and Conditions.
4. Mobile Information and Consent Protection
We do not share mobile information with third parties or affiliates for their marketing or promotional purposes. We do not sell, rent, or transfer SMS opt-in data or consent for another organization to use.
We may provide the minimum information necessary to service providers that help deliver and manage our own communications, including GoHighLevel, LeadConnector, and telecommunications providers. They may use that information only to provide those services to PMT, not for their own marketing. We may disclose information when legally required. These SMS restrictions control over any broader sharing statement in our other policies.
A referral, partner relationship, or another business submitting a contact does not provide SMS consent on that person’s behalf. We require consent directly applicable to PMT before enrolling a referred contact in an SMS program.
5. Other Sharing
We may share information with service providers that operate our website and CRM, professional advisors subject to confidentiality obligations, or authorities where required by law or necessary to protect rights and safety. Service providers are required to protect information and may not use it for their own purposes. Referral information is used to follow up on the referral opportunity, subject to the SMS restrictions above.
6. Cookies
We use cookies and similar technologies to understand website use and improve the visitor experience. You may disable cookies in your browser, which may limit functionality. We do not use cookies for advertising targeting or share cookie data with advertising networks.
7. Retention and Your Rights
We retain information as necessary for the purposes described here and applicable legal obligations, generally for our business relationship and a reasonable period afterward. Consent and opt-out records may be retained to document permission and honor requests not to contact you. Depending on your location, you may request access, correction, deletion, or limits on communications.
